REFER A NEW PATIENT
Our physicians see new patients on a referral basis only. To refer a new patient to our office, please fax patient contact and insurance information and all recent chart notes pertaining to the referring diagnosis. Our office staff will contact the patient to schedule an appointment.
For urgent referrals, please call our office at (541) 687-1712 and ask for the physician on call to discuss the case.

Sleep providers in our office include Board certified physicians; James Christon, Karthik Mahadevan, and Katsufumi Nishida. Jennifer McKinney and Martha Griffith have completed Physician Assistant (PA) certification and specialize in sleep medicine. Our sleep office provides treatment of a variety of sleep related problems including Sleep Apnea, Insomnia, Snoring, and CDL recertification.
